Before you choose
Know what to expect.
- Date one visit and identify the person receiving the handover.
- Check the facts needed to begin safely and respectfully.
- Give the next helper a factual visit note, not a diagnosis.
Practical limits
- This is an organisational handover, not medical, medication, safeguarding or emergency advice. Follow the person's authorised care instructions and relevant professionals.
- Share personal details only with authorised helpers. Do not record passwords, access codes or medical records on an unsecured copy.
- Fixed English print and fillable PDFs. Short text may still clip in a PDF reader; save, close and reopen a test entry before relying on it.
- The private Print Studio exports selected static pages. It does not copy typed entries, store files, send reminders or synchronise with another person.
No spreadsheet software is needed. PDF totals are manual. Separate fillable files accept text and checkboxes in a form-capable reader. They do not sync or save entries on this website.
